Multiple Line Text Fields

i working on application form inwhich 1 of text fields multiple lines (a whole page)  can please me want able have text field multiple line completing on-line if applicants want print out, lines them write on visible.

 

i have attached 2 page document.  if appicant complete form on-line, page 1 on screen, if applicant print document document page 2.

Error compiling for board Arduino/Genuino Uno.
